To replace a lost or stolen PVZP Private Health Insurance ID Card, Do not call any telephone number since a
Private Health Insurance ID can be replaced ONLY IN WRITING.

 

Please contact your authorized PVZP Agent.
If you did not purchase PVZP Private Health Insurance from an authorized agent or if your agent is unavailable, please  contact: office@VZPforForeigners.cz 
to arrange a replacement ID Card and schedule your next insurance renewal automatically. 
Kindly scan and send us a copy of:

- Valid Passport

- Expired PVZP ID Card if available.

- Confirmation of current Insurance plan.

To replace a lost or stolen VZP Public Health Insurance ID Card, Do not call any telephone number nor send any emails since a
Public Health Insurance ID Card can be replaced ONLY IN PERSON.

 

Please bring the following documents with you to your nearest VZP Client Center

- Valid Passport

- Temporary or Permanent Resident ID Card

- Future or Previous Employer Contract

- Expired VZP ID Card
